Party Administration of FMLA Leave Management (RFP No.  
725-HR-093) to Sedgewick Claims Management Services, of  
Memphis, Tennessee in the amount/rate of $3.39 per employee per  
month, for a total annual estimated administrative amount of  
$44,015.00. The contract duration is three (3) years with two (2)  
possible annual extensions. The funding source is the Human
